A somatic symptom disorder, formerly known as a somatoform disorder, is a category of mental disorder included in a number of diagnostic schemes of mental illness, including the Diagnostic and Statistical Manual of Mental Disorders, (latest version DSM-5) used by most mental health professionals in the United States. The diagnosis requires physical symptoms that suggest physical illness or injury – symptoms that cannot be explained fully by a general medical condition or by the direct effect of a substance, and are not attributable to another mental disorder (e.g., panic disorder). In people who have been diagnosed with a somatic symptom disorder, medical test results are either normal or do not explain the person's symptoms, and history and physical examination do not indicate the presence of a known medical condition that could cause them, though it is important to note that the DSM-5 cautions that this alone is not sufficient for diagnosis. The patient must also be excessively worried about their symptoms, and this worry must be judged to be out of proportion to the severity of the physical complaints themselves. A diagnosis of somatic symptom disorder requires that the subject have recurring somatic complaints for at least six months.

Een somatoforme stoornis is een psychische aandoening waarbij een persoon lichamelijke klachten heeft waarvoor geen somatische oorzaak (lichamelijke ziekte) gevonden is. De patiënt ervaart reële lichamelijke klachten, die niet ingebeeld zijn. De klachten zijn niet bewust of doelbewust nagebootst. Soms is de relatie met psychische oorzaken te leggen, maar niet altijd. Vaak is een ontregeling van het stressregulatiesysteem betrokken bij het ontstaan en in stand blijven van de klachten. De diagnose somatoforme stoornis kan niet gesteld worden als de lichamelijke klachten verklaard kunnen worden door een andere psychiatrische aandoening, bijvoorbeeld een depressieve stoornis of een angststoornis. In bepaalde gevallen wordt achteraf toch nog een somatische ziekte vastgesteld bij patiënten met een somatoforme stoornis. De getallen hierover lopen uiteen van 10-20%. Dit noopt bij de behandeling van dergelijke beelden altijd tot een tweesporenbeleid, waarbij zowel somatische als psychologische factoren worden meegenomen in de beoordeling. De bekendste somatoforme stoornis is waarschijnlijk hypochondrie, waarbij de patiënt doorlopend bang is een ernstige ziekte te hebben.
