A symptom (from Greek σύμπτωμα, "accident, misfortune, that which befalls", from συμπίπτω, "I befall", from συν- "together, with" and πίπτω, "I fall") is a departure from normal function or feeling which is noticed by a patient, reflecting the presence of an unusual state, or of a disease. A symptom is subjective, observed by the patient, and cannot be measured directly, whereas a sign is objectively observable by others. For example, paresthesia is a symptom (only the person experiencing it can directly observe their own tingling feeling), whereas erythema is a sign (anyone can confirm that the skin is redder than usual). Symptoms and signs are often nonspecific, but often combinations of them are at least suggestive of certain diagnoses, helping to narrow down what may be wrong. In other cases they are specific even to the point of being pathognomonic.

Ein Symptom (altgr. σύμπτωμα (symptoma) 'Zufall, Begebenheit', aus συν (syn) 'zusammen' und πίπτειν (píptein) 'fallen', davon das Substantiv πτῶμα (ptoma) 'Fall') ist in der Medizin bzw. der Psychologie ein Zeichen, das auf eine Erkrankung oder Verletzung hinweist. Es kann durch einen Arzt bzw. Psychologen erfasst (Befund) oder auch vom Patienten selbst erfahren werden (Beschwerde). Die Gesamtheit der aus einem Krankheitsprozess resultierenden Symptome ergibt das klinische Bild, die Symptomatik, auch Klinik genannt. Symptome können in subjektive (durch den Betroffenen wahrnehmbare) und "objektive" (von außen wahrnehmbare) Krankheitszeichen unterteilt werden. Letztere werden auch klinische Zeichen genannt. (Typische) Kombinationen von Symptomen werden auch als Syndrom bezeichnet.

Een symptoom is een kenmerk of klacht, behorend bij een bepaalde ziekte. Anders dan in het Engels, waar een symptom een klacht (beleving van de patiënt) is en een sign een objectief voor de arts waarneembaar teken van ziekte worden in het Nederlands met 'symptoom' beide soorten verschijnselen aangeduid. Er is wel voorgesteld voor 'sign' (objectief waarneembaar teken van ziekte) het Griekse woord semeion weer in te voeren, maar gangbaar is dit niet.

En médecine, un symptôme (du grec συμπίπτω, « rencontrer ») ou signe fonctionnel est un signe clinique qui représente une manifestation d'une maladie, tel qu'exprimé et ressenti par un patient. En général, pour une pathologie donnée, les symptômes sont multiples, et parfois il peut ne pas y avoir de symptôme : la maladie est dite dans ce cas asymptomatique. Inversement, un même symptôme peut très souvent être attribué à différentes maladies : on ne peut donc en général pas conclure automatiquement qu'un symptôme (par exemple, le mal de gorge) est dû à une maladie donnée (par exemple, la grippe) ; ce serait commettre le sophisme de l'affirmation du conséquent.
